Eligibility Requirements

  • Child must been diagnosed with cancer or relapsed in the last six months.
  • Child must be 21 years old or younger.
  • Child must have been living with parents/guardians in the United States at time of diagnosis and continuing during treatment.
  • Child must not have received an Orange Envelope previously.
  • Child must have a cancer diagnosis.
  • Child must be receiving treatment for cancer.
  • Child must be 21 years old or younger.
  • Child must have been living with parents/guardians in the United States at time of diagnosis and continuing during treatment.
  • Child must have received an Orange Envelope or been diagnosed more than six months ago.
  • You may not apply for an Orange Envelope at the same time as the All-Star Fund.
  • Child must be between 13-19 years old.
  • Child must have a cancer diagnosis.
  • Child must have been diagnosed more than one year ago.

Application Review and Notification

  • Changes to come in February 2023
  • Award notifications are sent to the primary email address and the social worker provided on the application. Please make sure to check your email regularly and the correct email address is on the application.

Application Details

  • Pediatric oncology social worker contact information (name, phone, and email).
  • For the All-Star Fund: a clear, complete, and readable copy of the bill you would like paid.

  • The application is only offered online.
  • It takes between 5-20 minutes to complete the application depending on the program.

  • We can pay rent, mortgage, car payments, utility bills and auto
    insurance, and more.
  • We can also provide a gift card for gas or groceries.
  • We cannot pay for car repairs, medical bills, prescriptions, credit
    card bills, cell phone bills, student loans, taxes etc.
  • We can only pay bills that are in the parent/guardian’s name.
  • We can only pay rent once we have received a W9 tax form from your
    landlord.
  • We can only pay bills that have clear and complete statements. If
    your statement is not clear, complete, and readable, your
    application may be denied.

All-Star Fund Award, Payment, and Use

  • If you are awarded a gift card, we will mail it to the address provided on the application.
  • If we are paying a bill, we will mail the payment directly to the creditor.
  • If you have received an All-Star Fund award of $800+, please wait 12 months between applications.
  • The annual gift card maximum is $500

Social Worker Instructions:

Our application system requires that each child has a unique login email address. This allows each family/social worker to enter their diagnosis and demographic information into their profile once instead of each time they apply for assistance. Once a family/social worker has filled out the child’s profile, they can complete an application for an Orange Envelope, an All-Star Fund, or PACK Experience.

  • Yes! Please follow the steps below:
    • Click “Sign Up” and use the family’s email address to create an account.
    • Create a password that you will remember.
    • Complete the profile and application for the family.
    • Click the gear icon in the upper right corner to log out of the family’s account so you are ready to apply on
      behalf of the next family.
  • Ideally, you will use the family’s email address for the login. If the family does not have an email address, you can
    use a fake email for the login (for instance to apply for Rosa Parks, I could use rosa@parks.com, rosaparks@gmail.com,
    or another formula that works for you). We will never send emails to the login address. Please make sure to enter your
    email address into the “Primary Email Address” field in this case.
